Helpful Information

  • I offer both in-person and telehealth visits, at the discretion of the client.

  • The office address is 2629 Main Street, Suite 130, Littleton, CO. 80120

    The office is located off the main street of downtown Littleton. There are many shops and coffee/tea stops off this street that we recommend checking out if you have any time before or after appointments.

    Street parking is available directly in front of and on streets around the building. You may also park in the permitted spots in front of the building, as long as you only stay the duration of our appointment.

  • The initial nutrition assessment is 75 minutes and $180. Follow-up appointments are 50 minutes and $130. Reduced fee options are available for those with financial need.

  • Revival Nutrition is in-network with most Cigna and Aetna insurance plans. If your insurance includes out-of-network benefits, we’re happy to provide you with documentation to support you in pursuing reimbursement independently, upon request. FSA & HSA cards are also accepted as a form of payment.

  • Our first appointment, the initial nutrition assessment, is devoted primarily to getting to know you and learning more about your history and goals.

    From here, we will discuss the ideal frequency of follow-up appointments with consideration for the nature of your care goals. Some clients work with us ongoing on longer-term goals, often related to recovery from an eating disorder or disordered eating; some clients for a handful of appointments on very concrete self-care and meal planning goals for general health & well-being.

    Sometimes we’ll spend our appointment planning meals and snacks or creating a meal plan, and often, when appropriate, we’ll spend time discussing your relationship with food and your body as a primary topic.

  • To request an appointment, please e-mail (rdkatieb@gmail.com), call or text (720-310-8674) to schedule. We will follow up to plan a 10-15 minute call to hear more about the support you’re seeking and make sure it’s likely we’ll be a good fit.

  • As able, please cancel at least 48 hours in advance of your appointment. For cancellations within 48 hours, a cancellation fee will be charged. This is necessary because a time commitment is made to you and is held exclusively for you. This is also a way we care well for ourselves within this work.

  • As of January 1, 2022 new legislation is in effect which requires healthcare providers to provide an estimate of treatment costs for patients who do not have insurance, or who are not using insurance benefits. This is called a Good Faith Estimate. Please read more about the No Surprises Act and your rights to a Good Faith Estimate here:

    “No Surprises Act”

    Congress recently enacted the “No Surprises Act” which went into effect January 1, 2022. The act is described as “new federal protections against surprise medical bills that take effect in 2022.” Surprise medical bills arise when insured consumers inadvertently receive care from out-of-network hospitals, doctors, or other providers they did not choose” (Learn more here). The goal is to protect clients from surprise billing by providing cost transparency.

    According to the new law, state-licensed or certified health care providers need to give a Good Faith Estimate of healthcare charges to every new and continuing client who is either uninsured or is not planning to submit a claim to their insurance for the healthcare services they seek. There are specific rules for what information has to be in that estimate and when it has to be provided. If you are a current or new client and have chosen to self-pay for services, we will provide this cost estimate to you detailing an estimated annual cost range for services.

    If you are currently working with us, you are already aware of the billing rates and your costs. At Revival Nutrition, we will always be transparent with you about the costs of the services we agree on together.
